Welcome to our Transition Learning Community!

 

This wiki began as a housing for all our workshops from the Vermont Agency of Education (formerly DOE) and Voc Rehab's (VR) "Conference in the Clouds". In addition to housing workshops and videos pertinent to transition planning for youth with disabilities, we are also going to use this site as a means for getting updates out to the field. 

 

Please take some time to get acquainted with our site. You'll notice folders along the right hand side. Those folders contain a variety of resources including examples of transition assessments, transition plan samples, FAQs and much more. If you scroll further down you'll see the quick links to the other transition resource websites for Vermont.  

 

The popular transition workshop videos are now in a folder along the right hand side.

The Vermont AOE Case Study Learning Project

 

Audience: All secondary teachers (general and special education), principals, curriculum coordinators, career and technical education center staff, and personalized learning plan/Flexible Pathways coordinators/advisors.

 

This resource is the result of several years of work aimed at answering questions educators have raised about the implications of personalized, proficiency-based education for students with disabilities. The goal of these case studies is to provide exemplars of the inter-relationship between a student’s personalized learning plan (PLP), IEP transition plan and the student’s flexible pathway to meeting proficiency-based graduation requirements (PBGRs). These case studies will provide examples of how our fictional Happy Valley High School executed the requirements and intent of Act 77, the VT Education Quality Standards, and IDEA. These case scenarios provide potential models that can be adapted to fit each school’s particular context. These models are not new requirements but rather tangible exemplars of how these plans might be developed to support rather than duplicate each other. Additionally this resource showcases a strategy for making PBGRs accessible for students with disabilities. The case studies can be found on our agency’s website.

Announcing the Vermont Graduation Readiness tool for IEP teams!

Description:

A long standing challenge for IEP teams in VT is how to determine the graduation date for students with disabilities, particularly students with developmental disabilities. Sometimes students graduate too soon and sometimes much later than necessary. VT AOE has coordinated an effort in developing a tool to help IEP teams decide on the date of graduation in a quantitative manner. Graduation readiness is more than just meeting the local proficiencies for graduation and this tool can help teams ensure their student has had preparation that is aligned with their specific needs to be successful after high school.

Join high school and middle school educators from across the state in the Secondary Transition vtMTSS Academy, an eight-week virtual learning experience focused on classroom instructional strategies supporting students to master research based college and career competencies such as goal setting, self-regulation, assertiveness, and conflict management.

Announcing Career Path’s April Intensive (April 18th-June 7th)

 

ReSOURCE is currently accepting applications for our Career Path April Intensive, designed for youth ages 18-24 who:

Have their high school diploma

Have some previous work experience

Are ready for full-time, permanent employment in manufacturing

 

Successful graduates are matched in full time positions with a local manufacturer.

During the program, students will gain hands-on experience in manufacturing, participate in professional development classes, earn industry-recognized certifications, and receive a $55 bi-weekly stipend.

My name is Dawn Breault and I’m a doctoral candidate at Plymouth State University and a full time transition coordinator for a public high school in NH. I’m looking to study/research the perceptions of full time transition coordinators specifically around their pre-service training. This is a multi-state research project - reporting on New England as a whole. I’m passionate about the field of transition and would like to interview 2-3 individuals that meet the criteria below.

 

 

Criteria for Transition Coordinator Pre-Service Investigation Research Project:

 

•             Individuals that work full time for a public school system as a transition coordinator. I'd like folks to be responsible for transition most of their day verses those that have dual roles (for example: transition and case management or transition and teaching).

 

•             Willing to speak with me for 30-45 minutes over skype or telephone if skype isn't an option.

 

•             Willing to speak with me regarding their perceptions surrounding their pre-service training - how it helped or hindered their ability to do their job.

 

•             Candidates can rest assured all data collected will be kept confidential and candidates will be coded - no names or identifying information would be used.

 

•             Their willingness to speak with me may help to provide the field with valuable information regarding what transition coordinators need in order to do their job well.

You are invited to a Professional Development Day for those working with Transitional Youth

 

This invitation is sent to you on behalf of AOE, UVM/CDCI, VFN, and DAIL;

 

We’d like to invite you to an event organized by the Agency of Education, DAIL, UVM/CDCI, and the Vermont Family Network. This is a professional development day to work on enhancing transition age services for youth with developmental disabilities. We are inviting regional interagency partners to attend together as a team to work on improving transition services both at the local and statewide level.

 

We are asking that your team consists of your VR Counselor, DDS Intake and DDS Supported Employment Coordinators, your Special Education Director, the vocational and Special Education case managers in schools with whom you specifically collaborate, and a parent and/or student representative.

 

The event is on 3/12/15, 9am to 3pm at the Barre Elks Club with lunch provided. We are also asking you to hold a snow date: March 19th; you can call (802) 760-8174 on March 12th to confirm if the event will need to be rescheduled due to weather.

 

We need your help to convene the local teams in two areas:

1)            As the VR and DDS partners, we would like to ask you to coordinate with one another in your plans to attend. In addition, we need your help to encourage educators to join us. Would you please work together to make sure that the rest of your local partners are encouraged to attend this event? Educators will also be receiving invitations from the organizing committee.

2)            Will you also invite a youth with whom you work and/or their parent/guardian to join us?

 

The goal is to bring local teams together to problem solve and work in unison. We once did these events quarterly, and many local teams have asked that we again, offer this type of development time together. It is understood that resources and time are tight, and that bringing these groups together is one tool to help the process and hopefully ease some of the pressure on everyone’s resources.

It's open house season for College STEPS! College STEPS is a non-profit that partners with colleges across Vermont to provide individualized support for students living with an autism spectrum diagnosis, a significant learning disability or an intellectual disability.

 

College Steps offers college support for three types of students:

1.     Fully matriculated students entering into, or already operating within, the college setting but for whom navigating college is challenged by social, academic, or independent living barriers.

2.     High school seniors interested in gaining a college primer experience during their senior year in high school.

3.     Returning college students, under the age of 26 years, interested in gaining a 2-year certificate in higher education.

The primary goal of the College Steps Program is to prepare students for meaningful careers and autonomy after graduation. Therefore, College Steps places a strong emphasis on self-advocacy, employment, and independent living skills. In my workshop titled, "Act 77: What does it mean for special education", there was a question about whether or not schools are required to shift away from credits with the new Education Quality Standards and I said I would follow up here. I am pasting in the below the paragraph from the EQS that addresses this.

2120.8 Local Graduation Requirements

"Each secondary school board is responsible for setting graduation requirements in accordance with these rules. Local graduation policy must define proficiency-based graduation requirements based on standards adopted by the State Board of Education. As required in 16 V.S.A.§261a(a)(1), it is the responsibility of the supervisory union board to ensure alignment in expectations for all students within a supervisory union. Schools may or may not use credits for the purposes of demonstrating that a student has met the graduation requirements. When used, credits must specify the proficiencies demonstrated in order to attain a credit and shall not be based on time spent in learning. Further, students may receive credit for learning that takes place outside of the school, the school day, or the classroom. Any credits earned must occur under the supervision of an appropriately licensed educator."

The College Steps Program partners with colleges and universities to support students of varying ability (e.g., autism spectrum disorders (ASD),  intellectual disabilities (ID), learning disabilities, etc.), ages 16-26, with their college pursuits. Thier target population includes young adults with the functional skills sufficient to thrive within a college environment but who remain challenged to succeed in a traditional college setting without additional supports. College Steps emphasizes inclusivity in the academic, social, and vocational learning programs we provide. College STEPS supports an atmosphere of academic dignity and integration for all students and believe in equal accessibility to college experiences for all.

The Vermont Assistive Technology Program (VATP) is actively recruiting Assistive Technology (AT) users and their family members to join their Advisory Council.  The VATP is the federal AT-Act program for VT and works state-wide, with people of all ages, to help improve access to effective AT tools for school, work, home, and play.  The mission of the VATP is to increase awareness & knowledge and to change policies & practices to ensure AT is available through all services to Vermonters with disabilities. The Advisory Council’s purpose is to advise the VATP in accomplishing its goals and mission with measurable outcome.  The Council is comprised of community members, and representatives from Vocational Rehabilitation, the Agency of Education, the Division of the Blind and Visually Impaired, the Workforce Investment Board, the Vermont Center for Independent Living, and other community organizations such as the Howard Center and Legal Aid.  We meet four times per year, two of which are in person, and two are conducted via telephone or web conferencing.  If you are interested in becoming a part of the VATP Advisory Council, please contact the program director, Amber Fulcher, at amber.fulcher@state.vt.us or (802) 871-3029.    We are looking forward to hearing from you!

If a student is due to graduate with a regular high school diploma or will be attaining the age when his/her entitlement to a FAPE ends, there is no requirement to conduct a special education evaluation. The responsible school district shall provide the child with a summary of the child’s academic achievement and functional performance, which shall include recommendations on how to assist the child in meeting the child’s post secondary goals.

Mansfield Hall Opens

 

College support programs begin this June.

 

Burlington, VT — Mansfield Hall, a college assistance and residential program for students with learning disabilities, mild autism, and other conditions, will open its doors for the first time in June 2013.  Mansfield Hall provides individualized training and skills development to help students not only flourish academically at the many nearby colleges in Burlington, Vermont, but also to adapt and thrive socially, emotionally, vocationally, and ultimately, independently.

 

“We are tremendously excited to welcome our first residents to Mansfield Hall this June.  The core of our mission is to help students with disabilities to successfully earn a college degree, so naturally we thought we would open the doors for the first time in August; but the initial response to our program and approach was so enthusiastic and immediate that we have decided to offer a summer program to a limited group. The deadline for priority applications is March 15, 2013,” said Dr. Perry LaRoque, President and Founder of Mansfield Hall.  “The Hall will serve as the residence and program center for roughly 20 college students during the school year.  It’s centrally located in downtown Burlington, so our students have easy access to many local campuses, including the University of Vermont, Champlain College, Chittenden County Community College, St. Michaels, and Burlington College,” he added.

 

In addition to Dr. LaRoque, the Mansfield Hall staff includes Executive Director Grant Leibersberger, MEd, MBA, as well as a residential director, a consulting psychologist, and educational director. The program is evidence-based and fosters students to become fully engaged in all aspects of their intellectual, artistic, physical and social lives in the context of a small, safe and supportive community. Mansfield Hall employs a holistic approach to help students reach their true potential and realize their ultimate goals through four guiding principles: Learning, Living, Giving, Engaging.

Michael Nerney is a consultant with more than 32 years of experience in education and substance abuse prevention. A former teacher and coach, Mr. Nerney also has served as director of the Training Institute of Narcotic and Drug Research, Inc. (NDRI), as a certified instructor in violence prevention and management with the Crisis Prevention Institute (CPI), and as consultant for two major television networks, having appeared on such programs as “20/20."

Mr. Nerney is an internationally known lecturer and consultant to numerous federal agencies. His particular areas of expertise include gender differences in communication, psycho-pharmacology, adolescent chemical dependency, relapse prevention, and managing violent incidents.

Mr. Nerney will discuss brain development and its relationship to future planning and decision making.

The U.S. Department of Labor has released "Skills to Pay the Bills: Mastering Soft Skills for Workplace Success," a collection of career development exercises and activities to help sharpen the communication and other "soft skills" of young workers, including those with disabilities. The curriculum covers communication, networking, enthusiasm and attitude, teamwork, problem-solving, critical thinking and professionalism. It was based on a survey of businesses to figure out the most important skills for young workers.

The Autism Transition Guide website is being developed by the Howard Center through a grant from Developmental Services. The steering committee is seeking photos and videos of young adults with ASD who have transitioned to employment or college and would like to share their story in a video, photos, or writing. Thanks, John. You have done a fantastic job with this site. i just veiwed the Transition resource tour video 1 and found it helpful (in spite of the fact that I am still on dial-up and it took a l-o-n-g while before I could actually watch it in its entirety). Also, re: the KU Transition Coalition seminars. I took them (with several colleagues) when offered two years ago and found them to be an excellent source of information. I have been telling other teachers at MAU about them and sharing the info ever since...and am hoping that some folks from MAU will sign up for this year's sessions.

Thanks, John, for the sample transition plans on Vt. forms posted 10/22. They help immensely!
I've got a couple of questions on Allison's plan: under Education and Training, is the Annual Transition Goal in Math the same as her IEP Goal? Also, under the List Transition Services in E&T, where it says "Visit three university campuses," is that a school, personal, or family responsibility? Similarly, under Employment, whose responsibility is it to arrange "job shadow experiences with children" and "part time employment in a position related to working with children." Since it's in the IEP it looks like it could be the school's responsibility to arrange the three university tours and job shadows. Thanks for your feedback and the great resources contained here!

Sorry for the delay in answering your question. Folks- keep the questions coming. That is part of why we have this wiki.

The annual transition goal underneath each post secondary goal is simply an annual IEP goal that relates to the student’s transition service needs. This annual goal should be directly linked to the post secondary goal. Can this annual goal which relates to transition services be a typical annual goal for math (in Allison’s example)? Yes but you don’t need to list it in 2 places. If it is only listed in the annual section it will need to have a clear indication that it is a transition goal. Also in the transition plan in the appropriate section it will need to indicate see annual goal x.

The transition services listed are services that will reasonably enable the student to reach her PS goals. In this example these are community experiences and employment training services.

Regarding the college visits- the LEA could include (where appropriate) web-based college exploration/virtual tours, visiting with college admissions reps (when they are at college “fairs” in VT), and inclusion in school-based college visits in the transition goals, with any actual visit to a campus left to the family.
